Intake Coordinator - Hospice Parker Jewish Institute's Community Health Services program is seeking an experienced professional to oversee the intake function for Parker's community programs-ensuring patients accepted to the program are appropriate and evaluated timely by home care and hospice staff . This is role is key in providing both quality patient care and actively working to expand our services, as Parker's Hospice and Home Care programs continue to grow. 1-2 years of prior as in a care coordinator, intake, admissions or community health role preferred Experience in a Home Care, Hospice or Community Health environment preferred Proficient in EMR software; familiarity with Netsmart is a plus
