The Intake Coordinator will directly educate a prospective patient through the intake process leading to enrollment with Always Compassionate Home Care. This person will work closely with referral sources, account managers, clinical team, and provider relations to ensure all tasks are efficiently completed associated with the enrollment process. The coordinator's direct job function would allow for a patient to receive the proper care they need and confirm all information about said patient has been recorded correctly.
•The position is based in-office/on site 5 days per week.
Responsibilities:
Serves as the initial point of contact for clients, families, and all referral sources Obtains initial authorization for care from respected parties
•pharmacies or insurance companies Proactively engages in organizing coverage for start of care Maintains consistent communication with referral sources Initiates contact/completes outreach to potential referral sources in the community/community partners Builds and maintains positive working relationships with referral partners Documents and reports to Supervisor when there is a change in client status, change in service requirement, change in employee assigned to case and any unsatisfactory performance
Qualifications:
Bachelors Degree in health-related field preferred Bilingual a plus Excellent time management skills/able to multi-task and priorities responsbilities as required Attentive to details, particularly with documentation Experience in working in a fast-paced health care/office environment Customer service experience Technically savvy

Salary:
$68,000
