A pioneer Federally Qualified Health Center, Open Door provides more than 300,000 patient visits annually and serves nearly 60,000 individual patients who might not otherwise have access. For more information, please visit our website at www.opendoormedical.org. Open Door is currently seeking a SBHC Medical Office Assistant in the Port Chester School District. The SBHC Medical Office Assistant works collaboratively with clinicians and multidisciplinary team members to support the patient care experience, ensuring alignment with organization policies, and School Based Health and patient-centered medical home standards.
DUTIES AND RESPONSIBILITIES PATIENT CARE SUPPORT SERVICES
Schedules well visit appointments in accordance with Relevant reports and parent/patient requests. Verifies need for appointments with the electronic medical record. Communicates with parents via phone and electronic medical record messages to inform them of the appointment, confirm appointment, and receive consent for appointment, vaccines and lab work. Prepares hall passes and travels throughout school buildings to bring patients for their appointments, handing out hall passes, escorting patients to and from the SBHC, etc. Documents all communication with parents and patients in the electronic medical record. Verifies health insurance on the day of the patient visit. Communicate with parents when health insurance information is missing or incomplete. Offers health insurance enrollment support to parents, and when parents agree, sends a telephone encounter to the Open Door insurance enrollment outreach team. Follows up on these encounters on a monthly basis. Responds to patient inquiries on a daily basis. Ensure that all documents, telephone encounters, requests for medical records, and labs are completed and/or answered on a daily basis. Keeps supervisor apprised of exceptions or problems with encounter forms and data entry tasks. Participates in cross training, as required, for the Coordinator and PCT job tasks; Performs SBHC outreach/enrollment functions as required; and registers new patients. Performs other duties as assigned.
CARE COMMUNICATION
Participates as part of the clinical team to assist patients through the process of the visit and through the health care system. Inform the Supervisor of any management or operating problems that may affect quality and standards of patient services. Demonstrates ability to function in a culturally sensitive manner within the organization's philosophy and mission. Fosters good communication between Open Door, the school, and the community by representing the organization in a professional manner throughout the school building, while greeting visitors at the front desk, and when answering the phone and making calls. Ensure timely communication with SBHC staff through individual, small group, or monthly SBHC team meetings, Teams chats, email, phone messages, electronic health record messages, etc. Makes follow up phone calls, pre-planned and post visit calls, to support the patient.
SUPPORTING ROLE
Supports the SBHC clinical team members by being very responsive to requests for help with scheduling appointments, confirming appointments, calling parents for consent, and inputting data and documents into the electronic medical record, etc.
QUALIFICATIONS EDUCATION
High school diploma or equivalent preferred
EXPERIENCE
Some experience in a customer service or related role desired SKILLS Fluent in English and Spanish Proficient computer use, and ability to work with electronic health records Ability to effectively communicate, both in writing and orally
WORKING CONDITIONS
Health centers located within school buildings Possible exposure to communicable diseases and other conditions common to a health center environment Maintains patient confidentiality at all times, as per Open Door policies and procedures. Prepares and maintains documentation in compliance with Joint Commission standards and applicable city, state, and federal laws. Frequent contact with patients, employees, and outside agencies. Requires adherence to Infection Control Standard Precautions.
PHYSICAL REQUIREMENTS
Involves long periods of sitting and moving about the work unit. Requires regular and recurring bending, lifting, stooping, and stretching, with some lifting.
