Description The Patient Service Coordinator, Centralized Para Placement Unit, is responsible for ensuring the initial placement of all needed paraprofessional services from a vendor contracted by St. Mary's Home Care, and the reassignment of cases to other paraprofessional vendors, whenever warranted. Will develop a good rapport with all vendors to ensure all cases are covered in an effective and timely manner, and are proportionally distributed among vendors. Works under general supervision.

ESSENTIAL DUTIES
  • Communicates to paraprofessional vendors, new and existing cases that require paraprofessional services, and works to have cases covered in a timely manner. Informs vendors of patients' needs per the Plan of Care.
  • Collaborates with vendors and assigned Home Care Team to ensure the level of service, timeframes, hours and days of service are accurately set for cases, and distributes communications to vendors and Home Care teams with the agreed upon information.
  • Ensures profiles are received from vendor agencies and ensures that profiles are in accordance with policies and procedures. Obtains signature from Supervisor of new vendor profiles. Assists with maintaining updated vendor profiles.
  • Informs Para scheduler to initiate paraprofessional schedule for initial cert periods in the Scheduling module in Cerner HomeWorks. Assigns the vendor(s) in Resource Assignment and transfers case to patient's Home Care Team.

Monitors changes in paraprofessional services and maintains log of each change by patient, and acts as centralized resource for clarification of the paraprofessional schedule for each patient for billing purposes. Follows-up with Home Care teams if changes noted on paraprofessional communication but not in Cerner HomeWorks or vice versa.

Acts as back-up support to Home Care Teams and scheduler for changes in paraprofessional scheduling when Home Care Team assigned personnel or scheduler is out of the office for extended periods of time.

Notifies and updates Senior management and management staff on the status of cases that have gone without coverage.

Participates in meetings, contributions to the improvement of procedures related to paraprofessional services and business operations.

Participates in special projects.

All other related duties as assigned. #SMSA123 Requirements Formal education required: Associates Degree preferably in Computer Science or the equivalent and/or related experience preferred.

Previous experience required:

Requires sufficient knowledge of hardware, software, and telecommunications operations, business requirements and area workflow process.

Strong knowledge in Microsoft Office applications.

Excellent consultative and communication skills, analytical ability, judgment, and the ability to work efficiently with management and staff.
