Elderwood is seeking a clinically experienced Admissions Coordinator (RN) to serve as a trusted liaison between hospitals, referral partners, patients, families, and our skilled nursing communities. In this role, you'll use your clinical expertise to evaluate referrals, coordinate admissions, and help ensure patients receive the right level of care at the right time.
