Patient Care Liaison (In-Person) Skilled Nursing & Post-Acute Services Build Relationships. Drive Growth. Make an Impact. Our health care organization is growing—and we're looking for a high-performing Patient Care Liaison who knows how to turn relationships into results. This is not a sit-back-and-manage role. This is for someone who thrives in the field, understands the pace of hospital discharge planning, and wants to own their market, drive census, and be recognized for it. If you've worked as a hospital liaison, clinical marketer, or in post-acute business development and you're ready for more autonomy, stronger leadership support, and real impact we want to talk to you. What You'll Do Own and grow your assigned Ohio territory across our Skilled Nursing Facilities Build and strengthen referral pipelines with hospitals, physicians, case managers, and payors Convert referrals into admissions through strong clinical positioning and responsiveness Identify gaps in the market and create strategies to increase census and capture rate Partner with facility teams to align on goals, messaging, and execution Mentor and support business development efforts across the region What Makes This Role Different High visibility + real influence on census and growth strategy Supportive leadership that understands post-acute and removes barriers Opportunity to lead and mentor , not just "drop off brochures" A company that is actively investing in growth, systems, and people What You Bring 2+ years in hospital liaison, healthcare marketing, or post-acute business development Proven ability to build referral relationships and drive admissions Strong understanding of skilled nursing and post-acute care Confidence, accountability, and the ability to work independently in the field A mindset focused on solutions, speed, and service excellence What Success Looks Like Increased referrals and conversion rates in your market Strong, consistent presence with key referral partners Measurable census growth across assigned facilities Being known as a trusted, responsive partner in your territory Ready to Make an Impact?
