Healthcare Community Liaison Job Summary Buckeye Commons Care is seeking a highly motivated and results-driven Healthcare Community Liaison to join our team in Kent, Ohio . As a Healthcare Community Liaison , you will be responsible for building relationships with referral partners, educating them on our services, and helping patients and families connect with the right level of post-acute care.
