Job Description
Concord Village Skilled Nursing & Rehabilitation is a beautiful newer facility with all private rooms on a shared campus with University Hospital in Lake County near Tripoint Hospital. We are seeking an experienced Admissions Coordinator to add to our Sales and Marketing Team.
Primary duties include:
Collecting information from incoming patients regarding services needed, insurance available, or financial status. Completion of admissions or insurance forms. Contacts insurance providers to verify coverage and obtain authorization for service. Uploading documentation to Point Click Care, prior experience essential. Prior experience with docusign helpful. Be a point-of-contact for families, discharge planners and other new resident referral sources. Answer their questions, and give tours of the facility. You'll work with Sales and Marketing to identify opportunities.Responsibilities:
Coordinate with hospital discharge planners, families and other referral sources; meet with families and clients to complete admission documentation via I-pad on Docusign. Process admission documentation and upload into Point Click Care. Ensure appropriate admissions paperwork, referral sources and other admissions data are entered into the automated referral system Ensure the client rooms are ready for admissions. Keep all resident information confidentialQualifications:
Previous experience in internal admissions at a skilled nursing facility highly preferred. Must have knowledge of Point Click Care, reimbursement programs including insurance contracts, Medicare and Medicaid, be able to operate a PC, have effective communication, organization and prioritization skills, and be skilled at public speaking.
