2988842660-2 Apply for this job now Job Description LifeStance Health seeks a Licensed Clinical Social Worker to provide outpatient therapy to children, adolescents, and adults in a patient-centered, mission-driven setting. Responsibilities include diagnostic assessment, individualized treatment planning, ongoing psychotherapy (in-person and via telehealth), risk assessment and crisis intervention, and coordination of care with psychiatrists, primary care, and community resources. Responsibilities Provide comprehensive biopsychosocial assessments and DSM-5 diagnoses for children, adolescents, and adults Deliver individual, family, and group psychotherapy in-person and via telehealth Develop, implement, and regularly update individualized treatment plans and measurable goals Conduct risk assessments, provide crisis intervention, and ensure appropriate safety planning Coordinate care with psychiatrists, primary care providers, schools, and community resources Maintain timely, thorough clinical documentation in the EHR in accordance with regulatory and payer standards Participate in case consultation, team meetings, and collaborative care initiatives Use evidence-based practices and outcome measures to guide and improve clinical care Support patient engagement, adherence, and recovery-focused, culturally responsive care Adhere to ethical, legal, and regulatory requirements, including HIPAA and professional standards Required Skills Clinical assessment and diagnosis (DSM-5) Individual psychotherapy Family and group therapy Treatment planning and documentation Risk assessment and crisis intervention Use of EHR/clinical documentation systems Telehealth service delivery Care coordination and collaboration Knowledge of evidence-based practices Outcome measurement and quality improvement
