We are growing our team and seeking an energetic, organized, and compassionate Certified Health Coach to support our patients and clinical programs. This is a unique opportunity for someone who enjoys patient coaching AND medical-office operations . The position combines health coaching, patient follow-up, EHR documentation, scheduling, administrative support, and social media/patient engagement. About the Role The ideal candidate is a people person who is also highly organized and comfortable with technology . You will work closely with our clinical team to help patients stay engaged with their individualized health goals while also supporting the day-to-day operations of our practice. What You'll Do Patient Health Coaching Provide one-on-one health and lifestyle coaching. Support patients with nutrition, exercise, sleep, stress management, and behavior change. Help patients establish and track SMART goals. Provide accountability and encouragement between clinical visits. Reinforce provider- and RDN-directed care plans. Follow up with patients through phone, portal, video, and other approved platforms. Identify concerns that should be escalated to the clinical team. Medical Office & EHR Support Schedule, reschedule, and confirm appointments. Assist with patient intake and onboarding. Document patient interactions and follow-up in the EHR. Update patient information, goals, care plans, and program status. Manage patient communications and administrative follow-up. Assist with forms, referrals, and other office responsibilities. Maintain HIPAA compliance and patient confidentiality. Technology Use EHR, scheduling, patient communication, and monitoring platforms. Assist patients with basic technology and telehealth needs. Utilize Microsoft Office/Google Workspace and other digital tools. Social Media & Marketing Support Assist with Wellpoint social media platforms. Create and schedule health and wellness content. Assist with Canva, newsletters, patient education materials, and program promotions. Help increase patient engagement and awareness of Wellpoint programs. Maintain a professional, clinically appropriate brand voice. What We're Looking ForRequired Certified Health Coach credential from a recognized health coaching program. Excellent communication and interpersonal skills. Strong organizational and time-management skills. Comfortable working independently and as part of a clinical team. Strong computer skills and ability to learn new technology. EHR experience or willingness to learn. Professional, dependable, compassionate, and patient-centered. Ability to maintain HIPAA compliance and confidentiality. Comfortable working both in-office and remotely . Preferred Medical office or healthcare experience. EHR and medical scheduling experience. Experience with health coaching or behavior-change programs. Knowledge of weight management, nutrition, lifestyle medicine, or chronic disease management. Canva, social media, content creation, or digital marketing experience. Experience working with NPs, physicians, RDNs, or other healthcare professionals. What Makes This Position Different? This isn't a traditional health-coach position. You'll have the opportunity to become an integral part of a growing healthcare team , working directly with patients while gaining experience in clinical operations, technology, care coordination, and health education. We are looking for someone who can move comfortably between: Patient coaching → EHR documentation → Scheduling → Patient follow-up → Clinical team support → Social media engagement Work Schedule This is a hybrid position with scheduled in-office and remote hours. In-office responsibilities may include patient visits, intake, coaching, team meetings, scheduling, and clinical-office support. Remote responsibilities may include virtual coaching, patient follow-up, EHR documentation, scheduling, administrative work, and social media/content management. Scope of Practice The Health Coach provides non-diagnostic health and lifestyle coaching and administrative support. The position does not independently diagnose medical conditions, prescribe treatment, interpret clinical findings requiring licensure, or modify medical treatment plans. Clinical concerns are appropriately escalated to the licensed clinical team.
