Job Description
Intake Coordinator -
NY Home Care Pay:
$24.68-$26.50/hour, depending on experience Monday-Friday, 8:30 AM-5:00PM Location:
Remote with occasional in-office days as needed Modivcare Personal Care Services is seeking an Intake Coordinator with direct home care experience to support Upstate New York referrals from initial intake through the first billable shift.What You'll Do:
- Manage Upstate New York home care referrals from initial intake through the first billable shift, taking ownership of the coordination needed to successfully start services.
- Verify Medicaid eligibility, payer information, authorizations, approved service hours, and required documentation to ensure cases are ready for service.
- Navigate NY Medicaid, MLTC plans, payer portals, and plan-specific requirements to process referrals, obtain necessary information, and resolve authorization or intake barriers.
- Coordinate caregiver matching and placement based on authorized hours, client needs, location, schedule requirements, caregiver availability, and other case-specific needs.
- Coordinate all start-of-care activities, including client and caregiver communication, confirmed schedules, required documentation, and first-shift readiness to ensure a timely and successful launch of services.
- Communicate and collaborate with MLTCs, CHHAs, LHCSAs, case managers, referral sources, clients, families, caregivers, and internal teams while maintaining accurate documentation and tracking each case through the first billable shift.
What You'll Need:
- 1+ years of direct home care experience in intake and/or care coordination required.
- Upstate New York home care experience strongly preferred, including experience with MLTC, CHHA, and/or LHCSA programs.
- Experience with NY Medicaid, payer portals, authorizations, and approved service hours required.
- Home care scheduling, staffing, caregiver matching, and start-of-care experience required.
- Experience with HHAeXchange or similar home care scheduling/EVV systems preferred.
- Bilingual English/Spanish strongly preferred.
- Strong communication, organization, follow-through, and computer skills.
