Care Coordinator / Patient Navigator (Managed Care) - Enhanced Care Management (ECM)
Location:
Alameda, CA Fully Remote Schedule:
Monday-Friday, 8:00am-5:00pm
Pay:
$43.51/hr Summary Now hiring a fully remote Care Coordinator / Patient Navigator to support a managed care Enhanced Care Management (ECM) program in Alameda, CA . This onsite role supports members who need help accessing medical and social services, understanding benefits, and navigating referrals. If you have experience in care coordination, discharge planning, authorizations/referrals, insurance benefits, or health plan member support , this is a strong opportunity to work in a fast-paced, member-centered environment with heavy communication, follow-up, and documentation. Responsibilities Conduct outreach and support orientation/assessment steps for members who may benefit from ECM services Coordinate care across members/caregivers, PCPs/providers, hospitals, and community resources Provide phone, email, and in-person support to help members meet care plan goals (in coordination with case managers as needed) Educate members and providers on benefits, eligibility, services, and referral processes Identify barriers to care and connect members to community supports (medical and social resources) Initiate follow-up to confirm next steps were completed and needs are addressed Serve as point of contact for assigned provider partners; support navigation of internal resources Assist with referrals/authorizations processes, documentation requests, and audit-ready tracking Maintain accurate documentation, data entry, and reports while following workflows and compliance standards Qualifications 3+ years of healthcare or healthcare customer service experience (managed care/health plan preferred) 1+ year of care coordination experience (outpatient clinic, office, home care, inpatient/discharge planning preferred) Working knowledge of medical and insurance terminology; strong documentation skills Proficient with Microsoft Office (Word, Excel, Outlook) and comfortable working across multiple systems Bilingual (Vietnamese/Cantonese/Mandarin/Spanish/Tagalog) is a plus
INDRS Pay:
$43.51 per hour
