Job Description
Skilled Nursing & Rehabilitation Facility Our skilled nursing and rehabilitation facility is seeking an energetic, relationship-driven Patient Care Liaison / Healthcare Marketer . This position is responsible for developing referral relationships, promoting the facility's clinical capabilities, and helping appropriate patients transition smoothly from hospitals and other care settings into our facility. The ideal candidate understands the local healthcare community, communicates confidently with clinical professionals, and is motivated by both resident outcomes and census growth. Key Responsibilities Develop and maintain strong relationships with hospital case managers, discharge planners, physicians, home health agencies, assisted living communities, and other referral partners. Regularly visit hospitals and healthcare organizations throughout the assigned market. Educate referral sources about the facility's services, clinical capabilities, availability, and admission criteria. Identify and pursue new referral opportunities to support short-term rehabilitation and long-term-care census growth. Respond promptly to referrals and coordinate with the admissions and clinical teams to determine whether prospective residents can be safely and appropriately admitted. Serve as a point of contact for prospective residents, families, referral partners, and community organizations. Conduct facility tours and clearly explain available services, amenities, and the admission process. Follow referrals from initial contact through admission and help address barriers that could delay placement. Maintain accurate records of referral activity, outreach efforts, outcomes, and market trends. Participate in community events, networking opportunities, and facility marketing initiatives. Monitor competitor activity and identify opportunities to strengthen the facility's position in the local market. Help ensure that all outreach and admission practices comply with HIPAA, Medicare and Medicaid requirements, and applicable healthcare marketing laws. Participate in occasional after-hours or weekend referral coverage as needed. Preferred Qualifications Previous experience in skilled nursing, post-acute care, healthcare marketing, admissions, business development, home health, hospice, or hospital case management is strongly preferred. Existing relationships with hospitals and healthcare professionals in Sumner County or the greater Nashville area are highly desirable. Clinical licensure—such as LPN, RN, or social work—is helpful but not required. Strong sales, relationship-building, presentation, and follow-up skills. Ability to understand clinical information and communicate effectively with both healthcare professionals and families. Organized, self-motivated, dependable, and comfortable working toward measurable referral and census goals. Ability to manage multiple referrals and priorities in a fast-paced environment. Valid driver's license, reliable transportation, and willingness to travel locally. What We Offer Competitive compensation based on experience. Performance-based incentive opportunities.
