Admissions Coordinator & Social Services Director Full-Time | Jackson, Tennessee Join Laurelwood Health Care Center

Responsibilities include:
Work with the Intake department on referral review, reviewing in a timely manner. If needed, verify all insurances and start authorizations. Maintain a working knowledge of community's practices related to payor and hospital contracts. Communicate with referral sources regarding referral status and communicate with residents and family members to complete paperwork. Work with the interdisciplinary team to prepare rooms, order equipment, and accommodate any other resident needs for potential admissions. Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department. Meet with and assist residents and their families with medically related services, grievances, or other placement needs to help maintain the highest quality of care. Complete assessments and participate in comprehensive care plans. Facilitate residents' safe transition upon discharge as well as facilitate transportation for appointments or other scheduled events. Maintain confidentiality of protected health information, including verbal, written and electronic communications. Perform all other assigned tasks as directed by the Administrator, Director of Nursing, or corporate team.
