Job Description

Bienvivir All-Inclusive Senior Health ("Bienvivir") is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly ("PACE"). PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

Bienvivir is currently accepting applications for the following position: Intake Coordinator LVN The Intake Coordinator under the direct supervision of the Intake Supervisor is responsible for the recruiting and screening of prospective participants for the program. The Intake Coordinator assists potential enrollees through the intake process, to include the Medicaid Eligibility worker (ME worker), and initiates the application process. The Intake Coordinator will also serve as a program-community liaison, with local community participant referral sources (i.e. hospitals, state agencies, churches, community health care providers, local businesses, etc.).
RESPONSIBILITIES
1. Works in conjunction with the Customer Service Representative in gathering all information needed to process an application for all potential participants. 2. Conducts initial screening of prospective enrollees. 3. Conducts a home visit for all prospective participants who want to enroll in the program and initiates the proper paperwork for the application process. 4. Arranges participants' medical appointments and maintains open communication with the Clinic Managers regarding the appointments. 5. Arranges for participants' days of attendance to the center by coordinating arrangements with the program transportation department. 6. Works with the IDT (Interdisciplinary Team) and the ME workers during the assessment period and participates as an integral member of the IDT to process the initial enrollment of recruited participants. 7. Attends morning meetings and PCP meetings three times a week or as needed and attends the I/A meetings as needed. 8. Provides referrals as needed to individuals who do not meet the basic qualifications, i.e. 55 years or older or reside out of the catchment area, withdraw application, or do not wish to proceed with application. 9. Initiates and maintains community liaison between agencies, hospitals, etc., as sources for participant referrals to the program and by scheduling presentations to such agencies as needed/requested. 10. Provides appropriate documentation for internal and external reporting purposes as required by the job. 11. Maintains a weekly activity report on all work and activities completed throughout the week. 12. Assists with TILE Recertification's for annual medical necessity as needed. 13. Communicates with TMHP with regards to all pending denials and provides additional information as needed. 14. Attends Medical Hearings for participants who appeal denials. 15. Maintains TILE Certification as required by the state of Texas. 16. Provides updates on potential participant's status throughout the entire application process with regards to medical and financial eligibility to the Intake Supervisor. 17. Tasks and responsibilities assigned by the Intake Supervisor.
QUALIFICATIONS/REQUIREMENTS
A graduate of an accredited institution as a Licensed Vocational Nurse (LVN) or a degree in Human Services.
KNOWLEDGE, SKILLS, AND ABILITIES
1. A dependable individual with a strong knowledge base and keen understanding, and appreciation of the elderly. 2. Strong organizational skills. 3. The ability to present the community philosophy of the PACE model for long term care for the elderly. 4. The skill to work collectively and communicate effectively with staff, community contacts, participants and family members. 5. Adaptable individual with the ability to work with the multidisciplinary team approach to care for the elderly. 6. Must achieve TILE certification prior or within one month of hire. 7. Bilingual (Spanish/English). 8. Must have reliable transportation, a valid driver's license, and auto insurance.
