•We are a growing hospice organization seeking an experienced and compassionate Director of Patient Care Services (DPCS) to lead our clinical team and oversee the delivery of high-quality, patient-centered end-of-life care. This position is ideal for a hands-on clinical leader who understands hospice, enjoys developing and supporting nurses, and is committed to maintaining strong clinical standards while helping a growing organization continue to improve. The Director of Nursing will report directly to agency leadership and work closely with the Administrator, interdisciplinary team, and clinical staff to ensure exceptional patient care, regulatory compliance, and continuity of services. Key Responsibilities Revised Key Responsibilities Oversee and coordinate clinical operations to ensure high-quality hospice care in accordance with Medicare Conditions of Participation and applicable State of Texas requirements. Provide clinical leadership, supervision, guidance, and support to RNs, LVNs, CNAs, and other clinical team members. Review patient eligibility, plans of care, diagnoses, clinical documentation, and patient progress to ensure appropriate and compliant hospice services. Oversee admissions and ensure new patients are admitted safely, efficiently, and in accordance with hospice requirements. Ensure appropriate staffing and clinical coverage, including coordination of admissions, routine visits, on-call needs, and changes in patient condition. Maintain a hands-on clinical presence and be willing to see patients, complete admissions, or provide coverage when clinicians are unavailable or additional support is needed. Participate in and provide clinical leadership for after-hours coverage, including availability as needed during nights and weekends to support patients, families, and the clinical team. Ensure interdisciplinary group (IDG) meetings, plan-of-care reviews, and required clinical conferences are completed according to regulatory requirements. Monitor clinical documentation for completeness, accuracy, timeliness, and compliance. Participate in hiring, onboarding, orientation, competency assessment, training, and ongoing development of clinical staff. Maintain ongoing clinical survey readiness and assist with regulatory surveys, audits, and corrective action plans as needed. Lead and oversee the agency's QAPI program, including identifying opportunities for improvement, monitoring performance indicators, developing and implementing performance improvement initiatives, and ensuring appropriate follow-through and documentation. Lead and oversee clinical components of infection control, emergency preparedness, and other quality and compliance initiatives. Monitor patient complaints, incidents, medication concerns, hospitalizations, and other clinical issues and ensure appropriate investigation and follow-up. Promote effective utilization and productivity of clinical staff while maintaining quality of care. Collaborate closely with the Administrator and leadership team regarding staffing, patient care, compliance, and operational needs. Serve as a clinical resource for staff and assist with complex patient-care situations, pain and symptom management, and changes in condition. Foster a positive, accountable, and collaborative clinical culture focused on excellent care for patients and families. Qualifications Current and unrestricted Registered Nurse (RN) license in Texas Bachelor of Science in Nursing (BSN) preferred Minimum of 2 years of hospice experience Minimum of 2 years of clinical leadership or supervisory experience preferred Previous DON, Clinical Director, DPCS, or similar hospice leadership experience strongly preferred Strong knowledge of Medicare hospice Conditions of Participation and Texas hospice regulatory requirements Strong understanding of hospice eligibility, plans of care, IDG processes, clinical documentation, and compliance Knowledge of pain and symptom management and end-of-life care Strong leadership, communication, organizational, and problem-solving skills Ability to effectively manage and develop a growing clinical team Current CPR certification Valid driver's license and reliable transportation

Pay:
$90,000.00 - $120,000.00 per year
