• $20 an hour 1 day ago Qualifications Teamwork Medical office experience Medical scheduling Administrative experience Home health Full Job Description Job Overview The Intake Coordinator is responsible for managing all incoming patient referrals and ensuring timely, accurate processing from intake through admission and billing readiness.

This role ensures full regulatory compliance with Medicare, Medicaid, and other payer requirements while coordinating effectively with clinical, administrative, and billing teams. Responsibilities

• Referral & Intake Management Receive, review, and process all incoming referrals for home health services Verify completeness of referral documentationincluding: Physician orders, Face-to-face (F2F) encounter documentation, Insurance authorization and eligibility Ensure all intake documentation meets federal, state, and payer regulatory requirements prior to admission Communicate with referral sources, physicians' offices, hospitals, and case managers to obtain missing documentation Coordinate timely patient admissions with clinical staff•Clinical Coordination Initiate and coordinate referrals for therapy services including: Physical Therapy (PT), Occupational Therapy (OT), Speech Therapy (ST) Collaborate with clinicians, Director of Nursing (DON), and Administrator to ensure appropriate care planning Ensure all services ordered align with regulatory and payer guidelines Work with primary care personal assistance services supervisors•Compliance & Documentation Ensure strict adherence to all regulatory compliance standards , including Medicare Conditions of Participation (CoPs) and agency policies Review patient charts for completeness, accuracy, and compliance prior to billing Maintain accurate and up-to-date intake records in the EMR system (Axxess, Vesta) Ensure every patient episode is complete, compliant, and billing-ready Participate in audits and support corrective actions when compliance issues are identified•Billing & Revenue Cycle Support Work closely with billing team to ensure timely claim submission Ensure documentation supports clean claims and compliance with payer requirements Assist in resolving billing issues including: Denials, Rejections, Missing documentation Support Accounts Receivable (AR) follow-up and collections processes Assist with Accounts Payable (AP) tasks as needed•Tracking & Reporting Maintain a referral tracking log for all incoming referrals Track and report referral sources and volume on a monthly basis Create and present monthly intake, referral, and compliance reports to leadership Monitor trends to support business development, compliance monitoring, and marketing efforts Track total skilled visits on a weekly basis for billing reports•Collaboration & Communication Work directly with the Administrator and Director of Nursing (DON) Serve as a liaison between intake, clinical staff, billing, and external partners Maintain excellent communication with all stakeholders Qualifications Experience in home health intake, medical office, or healthcare coordination preferred Strong knowledge of home health regulations, Medicare Conditions of Participation (CoPs), and insurance authorization processes Familiarity with EMR systems: Axxess, Vesta Strong organizational and multitasking skills Detail-oriented with strong follow-through Excellent verbal and written communication skills Ability to adapt to changing priorities, processes and workflows in a fast-paced environment Proactive problem-solver who can troubleshoot issues and prevent delays in intake and admissions process
