Assess client needs through interviews, standardized screenings, and collaborative discussions. Develop individualized treatment plans utilizing evidence-based practices, and coordinate care with multidisciplinary teams including psychiatrists, counselors, and other social workers. Facilitate access to community resources, assist clients in navigating public benefits, and advocate for client rights and well-being.
