Wellsential Health is currently seeking an Admissions Coordinator to support our team at Rio Grande City Nursing and Rehabilitation Center . Who is Wellsential Health? In this purpose-driven role, you will: As the Admissions Coordinator, you will support the admissions process from referral through admission while helping ensure a smooth transition for residents and families entering the facility. Handle admissions inquiries, conduct tours, and assist with admissions paperwork Coordinate resident transitions from hospitals or home settings into the facility Communicate with hospitals, discharge planners, social workers, physicians, families, and referral sources Assist with census development and referral relationship activities Support implementation of facility marketing and community outreach efforts Maintain referral, demographic, financial, and waitlist information within EMR systems Assist with screening referrals utilizing clinical and financial guidelines Promote facility nursing, rehabilitation, and specialty services Ensure timely follow-up on referrals and admissions inquiries Assist with customer service initiatives and admissions-related follow-up Participate in daily huddles, admissions meetings, and assigned on-call rotations
