Job Summary We are seeking a licensed LVN or RN to join our team as a Home Health Intake Coordinator. In this role, you will serve as the primary clinical liaison for incoming patient referrals. You will manage the intake pipeline, verify insurance benefits, coordinate care availability, and ensure a seamless, compliant transition of care for our patients. Responsibilities Referral Management & Screening:

Process inbound referrals from hospitals, rehab centers, and physicians via phone, fax, and electronic medical records. Meticulously screen patient records against agency criteria to ensure safe, appropriate care.

Data & Admission Processing:

Accurately enter clinical data, admission notes, and referral details into the electronic database and Excel tracking systems.

Insurance Verification:

Verify patient benefits and secure necessary initial authorizations for services.

Care Coordination:

Communicate referral details to the Clinical Coordinator team, track case assignments, and maintain an updated log of agency capacity.

Relationship Management:

Act as a professional point of contact for referral sources, including handling case declinations with gratitude and clear communication.

Qualifications & Requirements Licensure:

Active, unrestricted Texas Vocational Nurse (LVN) or Registered Nurse (RN) license.

Documentation:

Valid Texas Driver's License and auto insurance.

Experience:

1+ years of scheduling or intake experience in a Home Health setting is highly preferred.

Experience :

Nurse case manager skills to review a cross inpatient, outpatient, home health, hospice care, or clinic settings demonstrates versatility in diverse healthcare environments.

Skills:

Strong clinical knowledge of inpatient/outpatient processes, proficiency with MS Excel, and a strict commitment to HIPAA compliance. Familiarity with medical coding standards is a plus.
