Job description: PreferNP partners with skilled nursing and long-term care facilities throughout Rural Retreat and Abingdon to bring psychiatric and medical care directly to residents through onsite telehealth visits. We're looking for a dependable, take-charge Patient Care Liaison to be the in-person presence who makes every visit happen and the trusted connection between our patients, facility staff, and healthcare providers. This is a hands-on patient care role , not a tech support or reception position. You'll work directly with elderly residents, many of whom are managing mental health conditions, and you'll typically be the only PreferNP team member onsite. This role is built for someone who enjoys working independently, takes initiative, and confidently solves problems as they arise. If you're the type of person who walks into a busy facility, quickly assesses what needs to be done, and keeps everything running smoothly without constant supervision, you'll be an excellent fit. What You'll Do Welcome and build rapport with elderly residents, providing a calm and reassuring presence during their appointments Escort residents to their scheduled visits and remain present during psychiatric telehealth appointments as a supportive face Set up and manage the telehealth device (iPad) for each visit and independently troubleshoot basic technical issues Coordinate with facility nursing and administrative staff while communicating important patient information to our providers Schedule and track resident appointments according to their care plan and follow up on missed visits Take ownership of the onsite workflow by resolving day-to-day issues, keeping appointments on schedule, and ensuring a positive experience for residents and facility staff What We're Looking For Reliability above all. Clinical or patient-care experience, including: Medical Assistant (MA, RMA, CMA, CCMA) Certified Nursing Assistant (CNA) Licensed Practical Nurse (LPN) Patient Access Representative Medical Front Desk Other hands-on clinical experience is strongly preferred. Ability to stay calm, organized, and adaptable in a busy healthcare environment. Compassion and patience when working with elderly residents and individuals living with mental health conditions. Comfortable using tablets and video conferencing technology. Reliable transportation and willingness to travel between partner skilled nursing facilities in the South Hill area and surrounding communities. Local to South Hill, VA , or nearby areas. Pay, Raises & Guaranteed Hours Starting pay: $17-20/hour , depending on experience Opportunity for a raise after 90 days of dependable attendance and strong performance Training & Support We'll provide training on our electronic health record (EHR) system and introduce you to our workflow. After onboarding, you'll be expected to manage the day independently while having access to phone support whenever needed. $17.00 - $20.00 per hour Expected hours: 8.0 - 20.0 per week
