Job Description

Oasis Pavilion Nursing and Rehabilitation Center is seeking an SLP for our Skilled Nursing Facility. Oasis Pavilion is a 134-bed skilled nursing facility located in Casa Grande, AZ. Enjoy flexible scheduling options that meet your lifestyle needs. Essential functions 1. Assess, diagnose, and create personalized treatment plans for patients with speech, language, cognitive, and swallowing disorders. Use cutting-edge techniques and evidence-based practices to help patients achieve their fullest potential. 2. Plans, direct, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and/or swallowing or cognitive problems. Administers, scores, and interprets specialized speech tests. Instructs residents/patients, families, and caregivers to monitor speech and provide ways to practice new skills. 3. Completes required forms and clinical records in accordance with company policy and state and/or federal regulations. Documents in-patient records in an accurate and timely manner. Devises special tools and treatments for the specific needs of patients. 4. Collaborate with a multidisciplinary team, including physical therapists, occupational therapists, and nursing staff to deliver holistic care. Educate patients, families, and caregivers on treatment strategies and progress, fostering a supportive environment for recovery. 5. Document treatment plans and patient progress with precision and accuracy. 6. Provides comprehensive and compassionate therapy services in a sub-acute/skilled nursing facility setting. Works collaboratively with other rehab therapists and nursing staff in a supportive team-working environment. Maintains resident/patient privacy pursuant to HIPAA. Ensures punctuality and regular attendance for assigned shifts.
Qualifications Education:
Bachelor of Science degree in Speech Language Therapy, required.
Licenses/Certification:
Current license in Speech Language Therapy, as required by State law.
Experience:
Six months experience in a long-term care environment or equivalent, preferred.
Salary:
