The Speech Language Pathologist is responsible for providing rehabilitative therapy to patients upon referral for communication, cognitive and/or swallowing disorders following all regulatory and clinical practice requirements. This includes recovery, improvement in function and when possible, a return to a prior level of functioning. Clinical documentation will describe objective measures and justification for continued or discontinued services. Park Avenue Health & Rehabilitation Center is a 200-bed skilled nursing facility with a 5-star rating from Centers for Medicare and Medicaid Services. Qualifications:
Must be ASHA Certified (CCC) Must be currently registered/licensed in Arizona Skilled Nursing and/or Long-Term Care setting experience (preferred)
