Job Description

Golden Empire Nursing & Rehabilitation Center is a skilled nursing facility located in the heart of Grass Valley, CA. We provide nursing and therapy services to the geriatric population. We are looking for a Speech-Language Pathologist (CFY) to join our therapy team. This is a great opportunity for a new graduate to begin their career in a supportive skilled nursing environment with experienced professionals available to help guide you through your Clinical Fellowship Year. Position Responsibilities Provide speech, language, cognitive, and swallowing evaluations and treatment to residents. Develop individualized treatment plans based on residents' needs and goals. Provide therapy to residents with a variety of diagnoses and communication/swallowing disorders. Complete required documentation accurately and timely. Participate in interdisciplinary care planning and communicate with nursing, physicians, families, and other members of the care team. Monitor resident progress and modify treatment plans as appropriate. Provide education to residents, families, and facility staff regarding communication and swallowing needs. Maintain compliance with facility, state, and federal regulations. Qualifications Eligible for or currently pursuing ASHA Clinical Fellowship (CFY) . California Speech-Language Pathology license or eligibility for licensure. Strong communication and organizational skills. Compassionate and patient-centered approach to care. Ability to work effectively as part of an interdisciplinary team. New graduates are encouraged to apply!
