We are currently looking for a compassionate, organized, and team-oriented individual to fill the role of Per Diem Licensed Speech Therapist (ST) for the Santa Clara County You will work closely with our multi disciplinary team to ensure coordinated patient care. Our clinical and office staff in San Francisco work together harmoniously to deliver outstanding care. Responsibilities:
Speech Therapists will provide services within the scope of speech therapy, in accordance with the plan of care individualized for the patient, and in coordination with the Interdisciplinary team Follow the plan of care; provides treatment; evaluate patient's reaction to treatment and report any changes in his/her condition to the Physician, Case Manager and/or or Director of Nursing Maintain appropriate clinical records and/or notes and submit the records with organization policy Be responsible for following all company policies and procedures

$60.00
  • $75.00 per hour (depending on experience) Requirements Current ST License to practice in California
  • Required Home Health:
    1 year
  • Preferred Current CA Driver's License Coverage area: Santa Clara County
