Duties Assess patients' physical, occupational, or speech-language needs through comprehensive evaluations and clinical observations Develop and implement individualized treatment plans aligned with evidence-based practices and patient goals Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, feeding, and mobility to foster independence Coordinate discharge planning by collaborating with multidisciplinary teams to ensure seamless transition of care Document medical progress accurately using electronic health records (EHR) systems while adhering to medical terminology standards Provide therapy interventions for diverse populations including pediatrics, geriatrics, individuals with autism spectrum disorders (ASD), neurodevelopmental disorders, and disabilities Support early intervention programs and work with students with disabilities or special education needs in school settings Engage in case management activities to monitor patient progress, adjust care plans as needed, and ensure optimal outcomes Participate in inpatient or outpatient settings including skilled nursing facilities, acute care units, outpatient clinics, or home health visits Contribute to team meetings focused on patient care strategies, safety protocols, and quality improvement initiatives Skills Deep understanding of anatomy, physiology, and medical terminology essential for accurate assessments and documentation Proven experience working with individuals with neurodevelopmental disorders such as autism spectrum disorder (ASD) and other disabilities Skilled in assisting patients with ADLs and implementing discharge planning strategies effectively Strong case management capabilities coupled with experience creating comprehensive care plans tailored to individual needs Proficiency in electronic health records (EHR) management ensuring precise documentation of patient care activities Knowledge of evidence-based practices across pediatric, adult, geriatric, inpatient, outpatient, clinic, school-based settings as well as home health environments Ability to work collaboratively within multidisciplinary teams including physicians, educators, caregivers and other healthcare professionals Excellent communication skills for working with patients of all ages and backgrounds while supporting their therapeutic journey Familiarity with working with students with disabilities or those requiring special education accommodations Experience providing therapy for individuals with neurodevelopmental or neurodisability conditions such as autism spectrum disorders or post-acute care needs Qualifications Valid California license as a Physical Therapist (PT) or Occupational Therapist (OT) Experience working with children, adults, geriatrics, or individuals with developmental disabilities Home health experience preferred but not required Strong clinical, documentation, and communication skills Ability to work independently in a home-based setting Reliable transportation and willingness to travel within Ventura County Embark on a rewarding career path where your skills foster healing, independence, and improved quality of life.
