Per Diem / Contract / Fee-for-Service JTC Home Health Inc. is a growing, locally-owned home health company dedicated to providing high-quality, patient-centered care throughout the San Diego area. We are looking for a compassionate and skilled Speech-Language Pathologist (SLP) to join our team on a per diem, fee-for-service basis. This role offers exceptional flexibility and competitive compensation, allowing you to manage your own schedule while providing one-on-one therapy in the comfort of our patients' homes. Responsibilities Conduct comprehensive initial and ongoing speech-language pathology evaluations and assessments in the patient's home environment. Develop, implement, and document individualized treatment plans (POC) for patients with speech, language, cognitive, and swallowing disorders (dysphagia). Provide patient/caregiver education and training on home programs and compensatory strategies. Coordinate with physicians, nurses, and other members of the interdisciplinary healthcare team. Complete timely and accurate OASIS and documentation requirements using an Electronic Medical Record (EMR) system. Maintain open and professional communication with the JTC Home Health office team. Qualifications Master's degree in Speech-Language Pathology from an accredited program. Current and unencumbered California SLP license . Certificate of Clinical Competence (CCC-SLP) from ASHA required. Minimum of one (1) year of experience as an SLP, with at least six (6) months in a certified Home Health agency preferred. Knowledge of Medicare/Medicaid regulations and documentation requirements (OASIS experience preferred). Must have a reliable car, valid driver's license, and current auto insurance.
