The ideal candidate will be licensed by the Speech, Pathology, and Audiology Board and will play a vital role in designing, implementing, and evaluating individualized speech therapy programs for our participants. The ST will work closely with a multidisciplinary team to help participants enhance their communication and swallowing abilities, supporting their overall well-being.
Key Responsibilities Program Design :
Develop and oversee the speech therapy program, defining the responsibilities of the ST and any supporting personnel.
Assessments & Care Plans :
Conduct initial assessments, create individualized care plans in collaboration with participants, and complete quarterly and six-month evaluations to track progress.
Therapy Services :
Deliver direct speech therapy services as specified in participant care plans, with a focus on addressing communication and swallowing needs.
Team Collaboration :
Actively participate in multidisciplinary team meetings, ensuring a collaborative approach to participant care.
Documentation :
Maintain thorough records of assessments, treatments, progress notes, and any changes in participant conditions, in line with regulatory standards.
Family & Staff Training :
Educate family members and other team members on methods to assist participants with speech disorders and swallowing difficulties.
Qualifications Licensure :
Licensed Speech Therapist by the Speech, Pathology, and Audiology Board.
Health & Safety Requirements :
Must provide a health examination signed by a physician, a TB clearance dated within 12 months, and current CPR and First Aid certifications (or obtain within six months of employment).
Experience :
Previous experience in adult day health care or a similar setting is preferred.
Commitment :
Flexible to work hours that meet participant needs based on individualized plans of care. Hours available are between 9:00 am and 12:00. We are open Monday through Friday.
