In this vital role, you will provide comprehensive speech therapy assessments and interventions directly within patients' homes, empowering individuals of all ages to communicate effectively and improve their quality of life. Your expertise will help foster meaningful progress through personalized care plans, ensuring each patient receives the highest standard of treatment in a comfortable, familiar environment. Duties Conducts Appropriate evaluations,assesses home environments, I dentifies equipment needs relative to speech/language function. Identifies functional speech defects and establishes a plan of care to improve patient's function Administers speech therapy program utilizing specialized therapeutic technique and or equipment. Recommends communication devices.aids as indicated Confers with referring Physicians and other organization personnel to ensure coordinated and comprehensive care contributes to the toal plan of care. Educate patients and their families on exercises, strategies, and techniques to support ongoing improvement outside of sessions Maintain detailed documentation of assessments, treatment plans, progress notes, and patient records in compliance with healthcare regulations Requirements Current California Speech-Language Pathology License Issues by the California Speech-Language Pathology and Audiology and Hearing Aid Dispensers Board (SLPAHADB) Masters or Doctoral Degree in Speech-Language Pathology from Accredited program Certificate of Clinical Competence(CCC-SLP) from ASHA or Eligibility/CFY completion Current CPR certification Strong background in speech therapy techniques with a focus on patient care and communication disorders Knowledge of physiology, anatomy, and medical terminology relevant to speech-language pathology Ability to perform comprehensive patient assessments and interpret clinical data accurately Excellent interpersonal skills to build rapport with patients and their families in a home environment Join us to be part of a team that values your expertise and dedication!
