Upland Rehabilitation & Care Center is seeking a
PER DIEM SLP
house therapy team. We are a super busy 206 bed skilled nursing facility located in the heart of the Inland Empire. We have a busy sub-acute wing as well as outpatient programs, and our mission is to change and improve the way skilled therapy is delivered. Our beautiful facility has a spacious gym with great equipment and ADL spaces. Responsibilities Effectively screens/evaluates patients and/ with communication, cognitive or swallowing disorders and develops appropriate plan of care following all regulatory and clinical practice standards. Utilizes standardized assessments and evidence-based practice to support clinical interventions. Provides rehabilitative, skilled and medically necessary treatment intervention to patients with communication, cognitive and/or swallowing disorders per regulatory and clinical practice requirements. Performs other duties as may be assigned. Pay:
