Speech Language Pathologist (SLP)

  • Visalia, CA Salary range: $60
  • 65/hour BOE Linwood Meadows Care Center, a leading skilled nursing facility in Visalia, CA, is seeking a Full-Time Speech Language Pathologist (SLP) to join our compassionate and collaborative rehab team.
We are committed to providing exceptional care and helping our residents regain independence and quality of life.
Position Summary:
The Speech Language Pathologist is responsible for evaluating and treating residents with speech, language, cognitive-communication, and swallowing disorders. This role ensures timely, effective, and compassionate treatment aligned with physician orders and facility policies.
Key Responsibilities:
Evaluate patients within 24 hours or 1 business day of physician referral Develop treatment plans and obtain physician approval Administer treatments per the approved plan of care Supervise SLP Assistants and CFY clinicians in patient care activities Communicate effectively with the interdisciplinary team regarding patient status and progress Attend and participate in Patient Care Conferences, UR meetings, and staff in-services Document evaluations, daily notes, progress notes, recertifications, and discharge summaries per state and payer requirements Provide caregiver and staff education for post-discharge maintenance programs Ensure compliance with facility policies and the
State Practice Act Qualifications:
Bachelor's degree in Speech Language Pathology required; Master's or Doctorate preferred Current California SLP license in good standing Strong proficiency with electronic documentation (Casamba, PointClickCare preferred) Excellent communication, clinical reasoning, and organizational skills Ability to supervise CFY clinicians and collaborate with restorative staff
Physical & Work Environment Requirements:
Frequent standing, walking, pushing/pulling, hearing, and speaking Occasional lifting of 100+ lbs. and physical mobility for hands-on care Work environment is typically quiet to moderate in noise
