The Encore at Boca Raton Rehabilitation and Nursing Center seeks a PRN Speech Language Therapist (SLP) to provide evaluation and treatment for adult and geriatric residents in a skilled nursing and rehab setting. In this resident-centered role, you will assess speech, language, cognition, and swallowing; design individualized treatment plans; and deliver evidence-based therapy to support recovery, function, and quality of life. You will collaborate closely with an interdisciplinary team, educate families and staff, and document care in accordance with regulatory and facility standards. Responsibilities Conduct comprehensive speech, language, cognitive, and swallowing evaluations for short-term rehab and long-term care residents Develop and implement individualized treatment plans aligned with resident goals and clinical best practices Provide 1:1 and group speech therapy to improve communication, swallowing safety, and cognitive functioning Collaborate with an interdisciplinary team (nursing, PT, OT, physicians, social work) to coordinate resident-centered care Educate residents, families, and staff on communication strategies, swallowing precautions, and adaptive techniques Document assessments, progress notes, and care plans accurately and timely in the medical record Participate in care plan meetings and contribute to discharge planning and continuity of care Adhere to regulatory, ethical, and facility standards for skilled nursing and rehabilitation settings Required Skills Speech and language assessment Swallowing/dysphagia management Cognitive-communication therapy Treatment planning and documentation Skilled nursing/rehab clinical skills Interdisciplinary team collaboration Patient and family education Clinical instrumentation for swallowing (e.g., FEES/VFSS familiarity) Electronic health record (EHR) documentation Regulatory and reimbursement knowledge (SNF)
