Job Overview Join our dedicated healthcare team as a Speech Therapist in a long-term care facility, where your expertise will make a meaningful difference in the lives of diverse patient populations. In this role, you will assess, develop, and implement personalized speech and language therapy plans for residents. Your energetic approach and compassionate nature will foster a positive environment that promotes communication, swallowing safety, and overall quality of life. You will collaborate with multidisciplinary teams, families, and caregivers to deliver comprehensive care that meets each individual's unique needs. Duties Conduct thorough patient assessments to identify speech, language, cognitive-communication, and swallowing impairments across diverse populations. Develop and implement tailored speech-language therapy plans based on comprehensive evaluations and current best practices. Provide direct therapy services through in-person sessions or telehealth platforms, ensuring continuity of care for all patients. Collaborate closely with physicians, nurses, occupational therapists, physical therapists, and other healthcare professionals to coordinate patient care plans. Engage with patients' families and caregivers to educate them on communication strategies, progress updates, and home exercise programs. Maintain accurate documentation of assessments, treatment progress notes, and therapy outcomes using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems. Stay informed about advancements in speech-language pathology by participating in ongoing education related to anatomy, physiology, medical terminology, and specialized populations such as pediatrics and individuals with disabilities. Skills Strong knowledge of speech therapy techniques for geriatric patients with speech or language impairments. Expertise in conducting comprehensive speech and language evaluations utilizing standardized tools and clinical observations. Ability to design effective speech-language therapy plans aligned with patient goals and medical conditions. Excellent communication skills for collaborating with multidisciplinary teams and engaging with patients' families effectively. Familiarity with telehealth therapy platforms and delivering remote services confidently. Knowledge of anatomy and physiology related to speech production, swallowing mechanisms, and neurological functions. Experience working with individuals with disabilities or speech impairments across various settings including long-term care facilities and schools. Proficiency in using EMR/EHR systems for documentation; understanding of medical terminology is essential. Ability to adapt therapy approaches based on patient needs while maintaining a positive and motivating attitude.
