Responsibilities:

Perform comprehensive evaluations and OASIS assessments Develop and implement individualized care plans Treat patients with speech, language, cognitive, and dysphagia disorders Accurately document visits in accordance with Medicare guidelines Educate patients and caregivers on exercises and compensatory strategies Communicate effectively with physicians and care team members

Qualifications:

Master's degree in Speech Language Pathology Active Florida SLP license Home health experience preferred Knowledge of OASIS documentation is a plus Possesses and maintains current CPR certification Valid driver's license and reliable transportation
