Job Description

CenterWell Home Health is seeking a PRN Home Health Speech Language Pathologist to provide in-home evaluation and treatment for adult and senior patients. In this role, you will assess speech, language, cognition, and swallowing disorders; develop and implement individualized care plans; and deliver evidence-based therapies in patients' homes. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, document visits accurately, and support safe, independent living. Responsibilities Perform in-home evaluations of speech, language, cognitive, and swallowing disorders for adult and senior patients Develop and implement individualized, evidence-based treatment plans Provide direct speech-language and swallowing therapy in patients' homes Educate patients and caregivers on communication and swallowing strategies and home exercises Collaborate with an interdisciplinary home health team to coordinate care Document assessments, goals, progress notes, and outcomes accurately and timely in the EMRMonitor patient response, adjust treatment plans, and communicate changes to the care team Ensure compliance with clinical, regulatory, and safety standards for home health services Participate in case conferences and care planning meetings Support patient independence, safety, and quality of life at home Required Skills Speech and language assessment Swallowing/Dysphagia evaluation and treatment Care plan development Home health documentation/EMRPatient and caregiver education Interdisciplinary care coordination Geriatric rehabilitation Cognitive-communication therapy Time management and scheduling in the field Regulatory and compliance knowledge for home health
