Works in schools, hospitals and other healthcare facilities and works closely with physicians, psychologists, social workers, and other therapists.

Job Description
CenterWell Home Health, part of Humana Inc., seeks a compassionate Speech Language Pathologist to provide per diem in-home therapy for seniors and adults with chronic conditions. In this role, you will perform speech, language, and swallowing evaluations; develop individualized treatment plans; and deliver evidence-based interventions that help patients communicate better and remain independent at home. You'll collaborate closely with an interdisciplinary team, educate patients and caregivers, document care thoroughly, and contribute to a respectful, patient-first culture focused on quality and integrity.
