Responsibilities Conduct comprehensive speech and language assessments for patients of adult ages, including geriatrics Develop and implement tailored speech-language therapy plans that address individual needs and goals. Provide engaging therapy sessions utilizing evidence-based techniques for speech therapy, language therapy, voice, fluency, cognition, and swallowing disorders. Collaborate closely with parents, caregivers, educators, healthcare professionals, and multidisciplinary teams to ensure cohesive patient care. Document patient progress accurately in Electronic Medical Records (EMR) or Electronic Health Records (EHR), maintaining compliance with medical terminology standards. Educate families and caregivers on strategies to support communication development at home and in daily activities. Utilize platforms effectively to deliver remote therapy sessions while maintaining high-quality patient care. Qualifications Valid state licensure or certification as a Speech-Language Pathologist (SLP). Proven experience working with geriatric populations and programs. Strong knowledge of speech-language pathology techniques, anatomy, physiology, and medical terminology related to communication disorders. Experience conducting comprehensive speech and language evaluations for individuals with speech impairments or disabilities. Familiarity with working in home health and outpatient setting. Ability to collaborate effectively within multidisciplinary teams across various healthcare or educational environments. Proficiency in using EMR/EHR systems for documentation and patient management. Home health experience is highly desirable, demonstrating adaptability to in person service delivery models.
