Experience working in the acute care setting is required (minimum of one year experience practicing in the acute care setting is highly preferred). Candidate must have completed and possess Certificate of Clinical Competence (CCC's) and ASHA card. Position requires working a minimum of 2 weekend day shifts per month. Working one summer and one winter holiday or weekend around a holiday is required. Must work minimum of one of the 6 major holidays annually. Prior experience withEPIC EMR
is highly preferred but not required. During on-boarding process, some weekday availability will be required initially for orientation purposes (attend hospital's full day new teammate orientation session, attend rehab system's full day new teammate orientation session, attend a 2-hour safe patient handling class, attend a 2-hourEPIC EMR
training class, complete mandatory annual regulatory learning CBT modules, and orient to the department). Opportunity to pick up additional hours to assist with coverage of PTO, leaves of absence, and/or during times of increased hospital census/IP PT caseload. There is the opportunity to cross-train to a second hospital site should this be of interest. Individual must be flexible, reliable, collaborative, and be a self-starter. Must be able to deliver high quality, compassionate, and empathetic care. Pay Range $38.20- $57.
Major Responsibilities:
Interprets physician referrals and conducts a speech and language evaluation of patients with various speech, language, voice, cognitive and swallowing disorders. Establishes a written plan of care and implements individual treatment based on evaluation results, functional goals and by utilizing appropriate techniques. Monitors and evaluates outcomes and appropriately adjusts the individualized treatment plan based on the patient's response to treatment and provides status updates to physicians and other teammates. Identifies factors affecting patient's physical and psychosocial comfort and attends to these needs. Plans for and completes patient discharge including providing written home programs, patient/family teaching, referral for community services, transition planning within the continuum of care and ordering equipment as indicated/appropriate. Communicates and collaborates with appropriate stakeholders to assist in achieving optimum care for the patient. Completes all required documentation following department policy and accreditation standards with appropriate co- signatures, including patient evaluations, progress notes, team conference reports, and discharge summaries as appropriate.
