As a Speech Language Pathologist , you'll collaborate with an interdisciplinary care team to deliver evidence-based therapy, educate patients and caregivers, and support positive clinical outcomes throughout each patient's recovery journey. Your responsibilities will include:
Perform comprehensive speech, language, cognitive, and swallowing evaluations. Develop individualized treatment plans based on patient goals and clinical needs. Implement evidence-based interventions to improve communication, cognition, swallowing, breathing, and overall functional independence. Monitor patient progress and modify treatment plans as appropriate. Educate patients, caregivers, and family members on diagnoses, treatment strategies, and home exercise programs. Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation in accordance with agency policies and Medicare guidelines. Help reduce hospital readmissions through patient education and preventative care strategies. Participate in agency meetings, continuing education, and required in-service training. Qualifications Current Speech Therapy license in the state of employment. Minimum of one (1) year of Speech Therapy experience in an acute care setting. Home health experience preferred. Current valid driver's license, reliable transportation, and automobile insurance. Ability to meet the physical requirements of the position.
