Supportive DOR Duties and Responsibilities:
Responsible for the assessment of referred patients who require speech therapy services. Provide direct patient care to assess speech, language, cognitive-communication, and swallowing disorders. Evaluate patients' medical condition, functional capabilities, limitations, and potential for rehabilitation. Develop and implement individualized treatment plans with specific, measurable goals based on patient capacity and tolerance under the direction of the Physician. Establish realistic and achievable goals for patients. Perform swallowing evaluations and implement appropriate diet recommendations when indicated. Document and record patients' conditions; complete evaluations, weekly progress notes, recertifications, and discharge documentation. Educate patients and families regarding communication strategies, swallowing precautions, and therapeutic techniques. Collaborate with all disciplines to provide patient-centered treatment and care. Perform all other duties as assigned by the Director of Rehabilitation. Maintain compliance with confidentiality, HIPAA, and Federal Residents' Rights regulations.
Requirements/Qualifications:
Must have graduated from an accredited Speech-Language Pathology program. Must hold a valid state license to practice as a Speech-Language Pathologist. Certificate of Clinical Competence (CCC-SLP) preferred (CFY candidates welcome). Experience with long-term care and/or geriatric care preferred but not required. Experience with Case Mix Index and PPS scheduling preferred but not required.
