Responsibilities:
Develop and implement individualized speech therapy plans to address the unique needs of each resident Conduct assessments, evaluations, and treatments to improve communication, cognitive, and swallowing skills Collaborate with healthcare professionals to develop and implement comprehensive care plans Educate residents, families, and staff on health and wellness topics, disease prevention, and management options Maintain accurate and up-to-date records of resident progress and therapy sessions Participate in quality improvement initiatives and maintain compliance with regulatory requirements Speech Language Pathologist (SLP)
Requirements:
Minimum of 3 years of experience in a Skilled Nursing Facility (SNF) setting Knowledge of Point Click Care and Rehab Optima is preferred Professional attitude and appearance Ability to work effectively in a fast-paced environment and prioritize tasks
