Pay:

Visit Rates with

Mileage Reimbursement:

$63.00 - $100.00 per visit Discharge Without a

Visit:

$5.00 - $10.00 per non-visit discharge Transfer Without a

Orientation and In Service Hours:

$30.00 per hour Non taxed mileage reimbursement is paid at $0.50 per mile after 30 miles per day Visit Rates without

Mileage Reimbursement:

$68.00 - $105.00 per visit Provides services in speech pathology or audiology in accordance with the physician's plan of treatment. Accepts only patients referred by the patient care manager or designee. Demonstrates competency in the skills required by the agency on at least a biannual basis. Assists the physician in assessing the patient's level of functioning and the development (and revision, as necessary) of the plan of care by applying appropriate tests for speech, hearing and language disorders. Sets appropriate goals for the patient. Records and reports the patient's reaction to treatment and any changes in the patient's condition to the physician and/or patient care manager and case manager. Instructs and advises the patient, family and other health team personnel in the phases of speech pathology in which they may assist the patient. The goal is to teach and promote disease prevention for eventual patient & caregiver independence, utilizing agency materials as appropriate. Recommends hearing tests by audiologist when necessary. Checks hearing aides.

Requirements:

Active Michigan Speech Therapy license Reliable transportation Valid driver's license and auto insurance in your name as a driver Home health care experience preferred

Degree of Travel:

Home visits and office meetings as indicated. Must participate in necessary in-service programs. Must have reliable transportation and agency-required auto insurance.
