Full Job Description Speech-Language Pathologist, SLP In-House Subacute Rehabilitation Program Location:
Prestige Care Center of Plattsmouth Job Summary Prestige Care Center of Plattsmouth is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our in-house rehabilitation team on a Part-Time/Per Diem (PRN) basis. The ideal candidate will provide skilled speech-language pathology services to residents in our subacute rehabilitation and skilled nursing programs. This position offers the opportunity to work collaboratively with an interdisciplinary team focused on maximizing resident outcomes, improving quality of life, and promoting safe discharge to the community whenever possible. Responsibilities Evaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders. Develop and implement individualized treatment plans based on physician orders and clinical findings. Provide evidence-based interventions for dysphagia, cognitive deficits, communication disorders, and speech impairments. Complete standardized assessments and establish measurable treatment goals. Maintain timely, accurate, and compliant documentation in accordance with Medicare, Medicaid, state, and facility requirements. Participate in interdisciplinary team meetings, care plan conferences, and discharge planning. Educate residents, families, caregivers, and facility staff regarding communication and swallowing strategies. Collaborate with physicians, nursing staff, occupational therapists, physical therapists, dietitians, and social services personnel. Monitor resident progress and modify treatment plans as clinically indicated. Ensure compliance with all professional, ethical, and regulatory standards. Qualifications Required Master's Degree in Speech-Language Pathology from an accredited program. Current Nebraska Speech-Language Pathology license or eligibility for licensure. Current Certificate of Clinical Competence (CCC-SLP) preferred. Strong clinical, organizational, and documentation skills. Ability to work independently while contributing to a collaborative rehabilitation team. Preferred Experience in skilled nursing facilities (SNF), subacute rehabilitation, inpatient rehabilitation, or long-term care settings. Experience treating dysphagia, neurological disorders, stroke, dementia, and medically complex patients. Familiarity with Medicare Part A, Part B, and managed care documentation requirements.
