all over NH--- Job Summary We are seeking a dedicated and skilled Speech-Language Pathologist (SLP) to join our multidisciplinary healthcare team. The ideal candidate will possess a strong passion for improving communication and swallowing abilities across diverse populations, including pediatric and adult patients. This role involves delivering comprehensive speech and language assessments, developing individualized therapy plans, and collaborating closely with families, caregivers, and other healthcare professionals to optimize patient outcomes. The SLP will utilize evidence-based practices, including telehealth modalities, to provide high-quality patient care in various settings. Duties Conduct thorough speech and language evaluations for patients across different age groups, including pediatrics and geriatrics. Develop and implement personalized speech-language therapy plans tailored to each patient's unique needs and goals. Provide direct therapy services through in-person sessions or telehealth platforms, ensuring accessibility and continuity of care. Collaborate with families, caregivers, educators, and multidisciplinary teams to support patient progress and facilitate effective communication strategies. Maintain accurate documentation of assessments, treatment plans, progress notes, and discharge summaries using Electronic Medical Records (EMR) or Electronic Health Records (EHR) systems. Educate patients and their families about speech-language disorders, treatment options, and home practice activities to promote ongoing improvement. Stay current with advances in physiology, anatomy knowledge, medical terminology, and evidence-based practices related to speech-language pathology. Participate in team meetings, case conferences, and early intervention programs to ensure comprehensive patient care across settings such as schools or clinics. Experience Proven experience working with children in pediatric speech therapy or early intervention programs is highly desirable. Demonstrated expertise in conducting comprehensive speech and language evaluations for individuals with speech impairments or disabilities. Experience implementing speech therapy techniques for diverse populations, including those with developmental delays or neurological conditions. Familiarity with telehealth therapy platforms and delivering remote services effectively is preferred. Knowledge of working within multidisciplinary teams to coordinate care plans for patients with complex needs. Background working with geriatric patients on communication or swallowing disorders is advantageous but not mandatory. Strong understanding of anatomy, physiology related to speech production, and medical terminology relevant to patient assessment and treatment.
