$80 initial visit/$65 follow up visit/$70 discharge visit Now offering mileage reimbursement Gurwin Healthcare System is a full continuum of care providing extraordinary care and caring to the people that we serve. Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis. Sharing the campus is Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), and our Certified and Licensed Home Care Agencies. We are currently seeking a Speech Language Pathologist for our Home Care Division with adult experience who will embody the passion and commitment required to deliver the ultimate care for our patients. Per Diem (As needed) To preferably cover Nassau County and/or Eastern Suffolk but open to other towns as well!
EDUCATION AND EXPERIENCE REQUIREMENTS
Current New York Sate License to practice Speech Pathology. Minimum of two years experience as a Speech Pathologist. Home Health experience is preferred.
PHYSICAL WORK REQUIREMENTS
Ability to travel to patients' homes to make home visits. Ability to make clinical and managerial decisions within the scope of professional practice to perform speech therapy procedures competently and independently.
PRIMARY RESPONSIBILITIES
The Speech Pathologist is responsible for the planning and provision of speech/language and dysphasia rehabilitation services to patients in their homes and to work with patients and families to provide training and or assistive devices for speech/language and dysphasia rehabilitation.
Duties to include:
Reporting the patient's initial status, significant changes, therapy plans and goals, on-going status, and discharge plans. Providing the Agency with timely documentation including, but not limited to, assessments, plans, goals, visit care, summary reports, and billing/visit data. Instructing patients, families and team members regarding programs that enhance and/or facilitate care and follow-up. Evaluating the patient's social and architectural environment, making recommendations, and implementing them, as feasible. Complying with all regulatory criteria. Notifying the agency of visit schedule and changes. Providing professional leadership and clinical guidance and expertise to team members. Attending regular continuing education programs. Performing related duties, as necessary.
