Come work at Ten Broek! Position Summary Evaluates, diagnoses, and treats residents with speech, language, voice, cognitive-communication, and swallowing disorders. Develops and implements individualized treatment plans to improve functional independence and quality of life. Collaborates with interdisciplinary team members to ensure coordinated resident care. Ensures compliance with federal, state, and facility regulations. Key Responsibilities Clinical Assessment & Treatment Conduct comprehensive speech, language, cognitive, and swallowing evaluations. Develop individualized treatment plans with measurable goals. Provide skilled therapy services in accordance with physician orders and plan of care. Perform bedside swallow evaluations and recommend appropriate diet modifications. Implement evidence-based interventions for dysphagia, aphasia, dysarthria, cognitive impairment, and voice disorders. Monitor resident progress and adjust treatment plans as needed. Documentation & Compliance Complete evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries in a timely manner. Ensure documentation supports medical necessity and skilled services. Comply with CMS, Medicare Part A and Part B, Medicaid, and insurance guidelines. Participate in audits and survey readiness activities. Care Coordination Collaborate with physicians, nursing staff, dietitians, occupational therapists, physical therapists, and social services. Participate in interdisciplinary team (IDT) meetings and care plan conferences. Educate residents, families, and caregivers on treatment plans and home exercise programs. Provide recommendations for adaptive equipment and communication strategies. Quality & Safety Ensure safe swallowing practices and aspiration precautions. Train staff on proper feeding techniques and swallowing safety. Contribute to quality assurance and performance improvement initiatives. Maintain infection control and safety standards. Qualifications Master's degree in Speech-Language Pathology. Current, active state licensure as a Speech-Language Pathologist. Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association preferred. Experience in long-term care or skilled nursing preferred. Current CPR certification (if required by facility). Skills & Competencies Strong knowledge of dysphagia management and cognitive-communication therapy. Excellent clinical assessment and problem-solving skills. Effective verbal and written communication abilities. Ability to work independently and as part of an interdisciplinary team. Strong organizational and time-management skills.
