Speech-Language Pathologist (SLP) - Per Diem Certified Home Health Agency (CHHA) Community Services Division |
The New Jewish Home Area of Assignment:
Manhattan, NY Position Summary The New Jewish Home's Certified Home Health Agency (CHHA) is seeking dedicated and experienced Per Diem Speech-Language Pathologists (SLPs) to join our Community Services team. In this role, the Speech-Language Pathologist will provide home-based speech therapy services to adult and geriatric patients throughout Manhattan. Responsibilities include conducting comprehensive speech, language, cognitive-communication, and swallowing assessments, developing individualized treatment plans, and providing therapeutic interventions designed to maximize patients' communication abilities, swallowing function, safety, and overall quality of life. The ideal candidate will be committed to delivering high-quality, patient-centered care while collaborating effectively with an interdisciplinary healthcare team. Essential Duties and Responsibilities Conduct speech-language and swallowing screenings, evaluations, and assessments as prescribed by the physician. Develop and implement individualized treatment plans to address speech, language, voice, cognitive-communication, and swallowing disorders. Provide therapeutic interventions and rehabilitative services based on each patient's clinical needs and functional goals. Educate patients and family members regarding treatment plans, progress, home exercise programs, and discharge recommendations. Prepare and maintain accurate clinical documentation, including evaluations, progress notes, treatment records, and discharge summaries. Monitor patient progress and modify treatment plans as appropriate. Collaborate with physicians, nurses, therapists, social workers, and other members of the interdisciplinary care team to ensure coordinated, patient-centered care. Participate in case conferences, team meetings, and care planning discussions. Maintain required departmental and regulatory records in a timely and accurate manner. Assist in maintaining therapy equipment, supplies, and work areas. Participate in professional development and educational activities to remain current with best practices and advancements in the field. Perform other duties as assigned. Skills & Requirements Qualifications Required Current, unrestricted New York State license as a Speech-Language Pathologist. Experience working with a geriatric population. Availability to provide home visits throughout the Manhattan service area. Strong assessment, clinical reasoning, communication, and documentation skills. Ability to work independently while collaborating effectively with an interdisciplinary team. Preferred Master's Degree in Speech-Language Pathology or a related field. Previous home care or community-based healthcare experience. Experience treating adult and geriatric patients with speech, language, cognitive, and swallowing disorders. Skills and Competencies Strong clinical assessment and treatment skills. Excellent verbal and written communication abilities. Ability to establish effective therapeutic relationships with patients and families. Strong organizational and time-management skills. Commitment to delivering compassionate, patient-centered care.
