Helps patients improve their ability to make sounds or use language, or develop non-spoken means of communication. May specialize in working with patients of a certain age group, or treat speech problems resulting from a condition such as a stroke or cleft palate. Works in schools, hospitals and other healthcare facilities and works closely with physicians, psychologists, social workers, and other therapists.
Hybrid - Home & Community-Based Visits About the Position TriSource is seeking a compassionate and experienced Speech-Language Pathologist (SLP) to provide evaluation and treatment services to adults with acquired brain injuries and related speech, language, cognitive, communication, and swallowing disorders. This is a hybrid, community-based position that combines work from home with visits to clients in their homes and community settings. The ideal candidate will have experience working with adult populations, particularly individuals with neurological conditions or acquired brain injuries. Candidates must have reliable transportation and use their own vehicle for community visits. Mileage is reimbursed. Key Responsibilities Conduct comprehensive speech, language, cognitive, communication, and swallowing evaluations. Develop individualized treatment plans based on client needs and evaluation results. Provide direct and consultative speech-language therapy services in home and community settings. Treat communication challenges involving memory, expressive and receptive language, speech production, pragmatics, comprehension, problem-solving, and conflict resolution. Provide treatment for swallowing disorders and help clients strengthen the muscles involved in swallowing. Teach clients strategies to improve speech, voice, functional reading, and written communication. Develop and implement alternative and augmentative communication strategies for clients with limited or no verbal communication. Monitor client progress, conduct re-evaluations, and modify treatment plans as needed. Assess and recommend adaptive equipment and compensatory strategies. Provide education and training to clients, families, caregivers, and other members of the treatment team. Collaborate with physicians, case managers, rehabilitation professionals, and other healthcare providers. Participate in team meetings and contribute to individualized rehabilitation goals and discharge planning. Provide strategies and training to staff to support clients' treatment plans. Assist with vocational and avocational assessments, including task analysis and strategies to support clients in work and community activities. Complete evaluations, treatment notes, monthly reports, and other required documentation accurately and within established timeframes. Provide education and in-service training as appropriate. Maintain professional licensure, certifications, and continuing education requirements. Follow applicable professional, regulatory, and ethical standards, including ASHA guidelines, state practice requirements, HIPAA, DDD requirements, and CARF standards. Qualifications Master's degree in Speech-Language Pathology required. ASHA Certificate of Clinical Competence (CCC-SLP) required. Current state licensure as a Speech-Language Pathologist required. Experience working with adults required. Experience working with individuals with acquired brain injuries, neurological disorders, or other cognitive/communication impairments strongly preferred. Community-based rehabilitation experience strongly preferred. Excellent communication, organization, documentation, planning, and time-management skills. Ability to work independently while collaborating effectively with a multidisciplinary team. Valid driver's license, reliable personal vehicle, current auto insurance, and acceptable driving record required. Must be comfortable traveling throughout the community for client visits. Mileage is reimbursed.
