Overview We are seeking a dedicated and compassionate Per Diem Speech Language Pathologist to join our team. In this role, you will provide high-quality speech therapy services to patients. You will work collaboratively with families, and healthcare professionals to develop tailored treatment plans that enhance communication skills and improve overall patient outcomes. Responsibilities Complete all documentation required and in accordance with state and federal regulations and policy. Complete all billing required and in accordance with state and federal regulations and policy Perform patient assessments and evaluations to determine functional level and establish a therapeutic program Develop a therapy plan of care with the overall plan of care to include goals and discharge plan Within required time frames, document observations, patient's response to treatment, treatment programs, degree of progress and notable changes in patient condition per CMS and Hattie Ide Chaffee guidelines Communicate and coordinate patient care with other member's of the patient's health care team Provide patient, caregiver, and family training related to the patient's speech therapy program and its maintenance Provide speech therapy in accordance with physician's orders and accepted professional practices Assist the patient and/or family in procuring needed equipment and supplies Utilize appropriate hand washing, standard precautions, and personal protective equipment Attend staff meetings and patient conferences. Provide proof of attendance at mandatory staff in-services and/or training programs Maintain skills and training by attending continuing education and in-service programs Perform other duties as required Experience A Master's degree in Speech-Language Pathology from an accredited program is required. Valid state licensure or certification as a Speech Language Pathologist. One year experience in skilled nursing or medical facility required Strong assessment skills to evaluate speech and language disorders effectively.
