PRN JOB SUMMARY
Evaluates and treats residents and patients with temporary or long-term speech, hearing, swallowing, and cognitive disabilities in compliance with state and federal guidelines. Initiates preventive and rehabilitative therapy procedures as appropriate for the patient s care and safety. Develops and revises plan of care and discharge plan in conjunction with patient, family, physician, and care team to assist patient in reaching or maintaining a maximum level of independence. Evaluates the patient's response to therapy services, assesses service needs on an ongoing basis, arranges additional therapy services required, and obtains physician s orders for such services. Educates the patient, family, and care team members regarding disease process, self-care techniques and prevention strategies.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Screens and evaluates Resident and develops appropriate care plan. Under a physician's direction, provides direct patient / resident treatment based on established plan of care. Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems. Administers, scores, and interprets specialized hearing and speech tests. Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills. Completes required forms and documents in accordance with company policy and state and/or federal regulations. Maintains patient records in an accurate and timely manner. Devises special tools and treatments for the specific needs of patients. Other duties as assigned.
Documentation:
Documentation demonstrates compliance with Medicare, Medicaid, and other regulatory requirements. Documentation reflects notification of physician when patient's status changes and revision to the plan of care as appropriate. Documentation of visits reflects the patient's response to care and assessment of psycho-social needs through out the course of care. Clinical documentation with error rate less than 5% Documentation reflects complete assessment/reassessment of patient and includes goals, plan, and evidence of discharge planning.
Requirements:
JOB QUALIFICATIONS
Graduate of an accredited School of Physical Therapy/ Occupational Therapy/Speech Pathology with a Baccalaureate or Master s Degree. Must have a current licensure by the South Carolina State Board of the specified therapy and a valid CPR certification. Licensed in Speech Language Pathology as required by state law. Certification in clinical competency. One-year clinical experience preferred
OTHER REQUIREMENTS
Participates in (or conducts) company-sponsored training to safeguard against improper use and disclosure of resident's protected health information. Monitors and ensures compliance with company policies, procedures and state and federal law.
